Please contact the Health Department if you need a permit application not available here. All permits expire annually on December 31 unless otherwise noted.
All permits application require 10 business days to process.

Late permit renewal:
$100.00 for the first 30 days and $200.00 for each subsequent month
License / Permit duplicates:
$5.00 per duplicate permit
Photocopies:
$0.05 per page
